Emergency Contact Information and Medical Consent

Please ensure a medical consent form is filled in upon becoming a member or renewing your membership.

• As part of the membership process, all members are required to complete a medical form.

• A new form is completed every year. This will assist the adult volunteers to be able to deal with any medical emergencies.

• This information will be stored securely and only used in an emergency.

• Information will be destroyed after 12 months (end of membership period).
